Patient Group Meeting

The next Patient Group meeting will be held at the surgery at 6:30 on Wednesday 6th December 2017. Everyone welcome.

This meeting provides an opportunity for patients to understand and influence the way we deliver services to patients.

Please note: The Patient Group Meeting is not a forum for individuals to raise personal issues. The Comments, Concerns and Complaints procedure should be followed in such instances.

New Patient Registrationregister_now

To register with Garswood Surgery, please call into the surgery and collect a GMS1 Registration form and a New Patient Health Questionnaire for each person registering. Then complete these forms and hand them back to the receptionist.

If you would like to join the practice you should ideally bring your medical card with you when you come to register, however, if this is unavailable we can give you a form to complete. Registration is easy and takes only a couple of minutes.

Please note, it is not necessary for every member of a household to complete forms individually. Any adult member of the family can register any other family member and sign the registration applications on their behalf.

*Please note that we are unable to provide newly registered patient with an urgent appointment until at least 48 hours after their registration has been accepted.*

We will register your details on our clinical database you will receive a letter from Central Operations confirming your registration has been accepted. We will request your medical records from your previous GP surgery at this point, but these can take up to 6 weeks to arrive at the surgery. If you take any regular medication, please bring in your repeat order slip in with your registration form as confirmation of your medication, and these will be added to your clinical record. Please note that the GP may wish to see you prior to your medication being issued.

As a patient you have the right to express preference as to whom you wish to see, however, if you require an appointment and your preferred GP is not available you will be offered an appointment with another doctor. If you still wish to see your preferred GP you may have to wait longer.
